How to Apply Online

You need to download the form through the left navigation menu. After that, you will be asked to upload the needed files through the “Upload Document” link located on the applicant home page. The last step will require you to be personally present at the Passport Seva Kendra (PSK) or the Mini Passport Seva Kendra (Mini PSK). Also, you must schedule an appointment with the PSK or the Mini PSK that is associated with the jurisdiction of the applicant’s current and valid address.

When you chose to decide to have an Online Appointment, it is mandatory to go to PSK to commence further passport processing. Only people who have medical or emergency cases are able to go to PSK without any appointment. Please be reminded that with these cases, such service will be provided with discretion from the PSK in charge.
